Lanzini has just shown exactly what West Ham have missed from the start today as he skips past his man to set-up a decent opening but his pass doesn't find a team-mate.
90' +3
Amidst the craziness, there's five minutes of added time. West Ham are taking as much time as they can on the ball, with Swansea looking deflated.
Swansea conceded a late goal in the defeat to Watford last week and Sakho's goal looks to have condemned them to another gut-punching loss. Paul Clement's men certainly don't deserve to be heading back to Wales empty handed.
90' +1
D. Sakho gets yellow.
Diafra Sakho took his shirt off in the celebrations and is booked, though he probably won't care.
90'
D. Sakho has scored a goal for West Ham United! Assist by A. Masuaku.
West Ham have stolen it! That could be a huge goal for the Hammers and Slaven Bilic, with Diafra Sakho smashing home at the far post. It's the substitutes that combine as Masuaku drives a low ball across the face of goal, putting the ball on a plate for his team-mate.
Sakho's goal may take the headlines but it's Masuaku who deserves the plaudits, providing an exceptional assist just a few minutes after providing the ball that saw Carroll head onto the bar.
87'
S. Clucas enters the game and replaces M. Olsson.
Final substitution for Swansea sees Sam Clucas come on for the final few minutes, with Olsson replaced.
86'
Off the bar! Andy Carroll is inches away from giving West Ham the lead as Masuaku's ball into the box bounces up to the big striker and his header leaves Fabianski beaten. It's the closest we've come to a goal this afternoon and some home fans have seen enough.
85'
Diafra Sakho hasn't seen too much of the ball since coming on but he's flagged offside there, with Antonio's decision to pass perhaps being te wrong one as there was space to drive into.
84'
After showing some improvements, West Ham are now set back in their ways of struggling to retain possession or create any chances of notes. The home fans are not on their side and a last-gasp goal from Swansea could make for a toxic atmosphere.
81'
As it stands, Swansea are on course for a fourth clean sheet on a road this season. They're yet to concede a goal on their travels this term and their three clean sheets on the road have already equalled their tally from the previous two campaigns combined.
80'
Renato Sanches, on a yellow card, has just kicked the ball away in frustration having failed to keep it in play. There's no action from the referee but he needs to be careful, having earlier clashed with Winston Reid.
Minute
Description
78'
D. Sakho enters the game and replaces J. Hernández.
A. Masuaku enters the game and replaces A. Ayew.
West Ham make a double change in an attempt to snatch all three points. The first change sees Javier Hernandez replaced by Diafra Sakho, a change that is met by a rapture of boos from the home support.
The other change from Slaven Bilic won't create as many talking points, unless it leads to a goal. Arthur Masuaku has come on to replace Andre Ayew.
76'
Swansea, for their part, have looked bright today but seem to be a spark short of firing properly. The same can't be said for West Ham though, who've looked short of ideas for the majority of the game.
75'
Swansea enjoying a decent passage of play and Martin Olsson has just flashed an effort from outside the area wide of Joe Hart's left-hand side post, following superb build up play from Paul Clement's side.
73'
West Ham are at least starting to ask questions of Swansea's defensive line, as Fabianski is again required to rise to collect a threatening cross.
71'
Heading into the final 20 minutes and the match is delicately poised. Difficult to tell which side seems more likely of breaking the deadlock but West Ham have certainly improved since the break.
69'
Roque Mesa enters the game and replaces L. Britton.
Swansea now take off their captain Leon Britton, following West Ham's decision to take off Noble, and £11m summer signing Roque Mesa enters the fray.
68'
Well, Fabianski looks fine as he rises well to clutch a lofted ball out of the air.
67'
Fabianski tells the bench he's able to continue and play commences. Swansea will certainly keep an eye on him.
63'
Carroll again the target for the Hammers with a long ball and he clatters into Fabianski, with the goalkeeper than requiring treatment. It's a big clash and a crunch landing for the Polish stopper, which isn't nice to see. Kristoffer Nordfeldt is warming up as a precaution but it's not looking good for Swansea.
62'
M. Lanzini enters the game and replaces M. Noble.
It is Noble that makes way though, which shows real attacking intent from the Hammers. Lanzini will play centrally alongside Kouyate, a duo that both like to get forrward.
60'
West Ham's home support can be heard now as Manuel Lanzini warms up, which is exactly the sort of change that the Hammers need. In all honesty, their worst player in midfield this afternoon has been Noble but it would take a bold move from Bilic to substitute his captain.
59'
West Ham are looking increasingly threatening but they're still a ball short of crafting open a decent opportunity. Zabaleta is starting to become an asset in the opposing half but Noble sloppily gives the ball away.
55'
A. Carroll gets yellow.
C. Kouyaté gets yellow.
First yellow card for West Ham as Kouyate dives into a challenge on Sanches, with both feet raised off the ground. A red card would certainly have been harsh but it's the sort of challenge he needs to be careful with.
Two bookings in quick succession for the Hammers as Andy Carroll kicks out to bring a halt to Tammy Abraham's darting run. From the resulting free-kick, Renato Sanches sends a shot flying into the sky.
52'
How didn't that go in? Antonio whips an inviting delivery towards the far post where Carroll slides towards the ball but fails to connect with the cross. It's West Ham's best chance since the opening few minutes of the first-half and it's resulted in a roar of support from the home faithful.
51'
West Ham do go direct with their corner as Cresswell's delivery finds the head of Carroll but his header is blocked by Abraham, who requires treatment.
50'
Understandably, the home fans are getting increasingly frustrated and they've made their feelings known as Cresswell elects to take a free-kick short rather than pumping a ball into the box towards the head of Carroll.
49'
L. Britton gets yellow.
Second booking of the game for Swansea and it's handed to Leon Britton.
48'
If anything sums up West Ham today, it's Mark Noble on the ball there. Under no pressure at all, he slices an attempted clearance and somehow sends it back towards goal. Hart is there, thankfully, preventing a gift of a goal.
47'
Winston Reid concedes an early corner for Swansea, slicing away his clearance, and Tom Carroll goes over to take. Abraham wins the initial header, putting the ball back across the six-yard box, but West Ham clear it away.
46'
L. Fer enters the game and replaces W. Bony.
One substitution at the break from Swansea as they bring on Leroy Fer for Wilfried Bony, in what's perhaps a surprising move. Ayew looks to have moved up front, with Fer slotting into midfield.
45' +1
One minute of added time at the end of the first-half, which has been quite poor.
45'
Back underway at the London Stadium, Swansea get us underway and are now kicking to the right.
44'
Swansea have dipped in the last few minutes and West Ham look to be more threatening. The visitors look like they need the break, which is just minutes away, whereas West Ham won't want to hear the half-time whistle whilst they're on this spell.
42'
It's a delivery into a decent area but West Ham's defensive line is good, not allowing Swansea to latch onto the head of it and call Hart into action.
41'
West Ham's only attacking outlet is going long to the head of Carroll and it's not really working. Meanwhile, Jordan Ayew isn't happy with a challenge from Pablo Zabaleta and the pair get a talking too. It's a deep free-kick for the visitors.
39'
Another long ball, this time directly into the box, finds the head of Carroll but he's unable to provide the power or direction necessary to test Fabianski. The Hammers have certainly improved in the past five minutes and are now enjoying a decent period on the ball.
38'
That chance came around following a long ball from Antonio to Carroll, switching sides, showing how useful the former Liverpool man can be- but he needs the ball his way to create something.
37'
A shot on target for West Ham but not really one that's too threatening. Carroll's cross into the box is headed goalwards by Andre Ayew but it's an easy save from Fabianski.
35'
Just over ten minutes until the brwak but it's already looking like West Ham need to change something. Manuel Lanzini, who's played just 45 minutes of Premier League football this season due to a knee injury, is the ideal option for Bilic in the second-half.
32'
West Ham are really struggling to get going here. Chicharito has barely had a sniff of the ball and the majority of Carroll's contribution has been defensive. The issue is the Hammers aren't keeping the ball long enough to find either of their two strikers, either kicking the ball long or simply conceding possession.
30'
Renato Sanches gets yellow.
The Bayern Munich loanee is pleading his innocence but it's a deserved booking, he really can't have too many complaints.
29'
Sanches has just fouled Cresswell with a late challenge, showing his studs to the defender's ankle but all the force came from Cresswell's kick.
28'
Another poor cross from Swansea, Carroll the cuplrit this time, concedes possession yet again. They need to ask more questions of the West Ham defence, particularly as the Hammers' back-line isn't looking too sturdy.
26'
Renato Sanches is seeing a lot of the ball for Swansea, which is dangerous for the Hammers. They've given the Portuguese international too much space on the ball and they could live to regret it- although his cross into the area there is far too deep, passing over Hart's net and behind for a goal kick.
25'
Free-kick for Swansea out wide but Carroll's delivery can't be capitalised on as Jordan Ayew is judged to have fouled Carroll, giving a free-kick to the defending side.
23'
The possession stats will certainly make for frustrating viewing for West Ham fans, they've had just 30% in the opening exchanges.
22'
Swansea certainly look like the side more likely to score at this stage and Renato Sanches is growing into the game. He shifts a short pass to Carroll and the midfielder's shot is blocked.
20'
Bony's effort on goal a few minutes ago seems to have sprung the Swans into life, as they're now seeing a lot more on the ball whilst still frustrating the Hammers by quelling any attacking threat the hosts craft.
17'
Cheikhou Kouyate is down near the half-way line after what appeared to be a foul from Tom Carroll but referee Roger East refused the calls for a free-kick, then stopping play moments later to allow treatment for the Hammers midfielder.
15'
First effort on goal from Carroll for the Hammers but it's a tame one, as his volley bounces towards goal and is comfortably collected by Fabianski.
First chance on goal for Swansea as Wilfried Bony turns his marker and fires a strong effort at goal from outside the box, which Hart dramatically pushes away with a diving save to his left.
12'
Foul by Cresswell on Renato Sanches sees Swansea retain possession, in what's their best passage of play in the opening period. However, the West Ham defence remains strong and refuses to give the visitors an opening.
11'
Decent link-up play between Javier Hernandez and Carroll sees the latter charge down the wing on the right but the big striker is unable to create anything- the Hammers would have certainly preferred that passage of play to have unleashed the Mexican, with Carroll instead running into the box.
8'
Swansea are slowly establishing themselves in the game now and are comfortable to knock the ball around their defensive line. So far, they've been unable to show any signs of threatening Joe Hart's goal.
7'
Andy Carroll has a strong record against Swansea, scoring five goals in six games against the Welsh side, and he's already proving to be a handful for the Swans' back-line this afternoon.
4'
First chance of the game falls the way of the hosts and it's Aaron Cresswell with a high cross into the box which Michail Antonio volleys into the ground, the bounce making it difficult for Lukasz Fabianski but the Polish stopper palms the effort away. Andre Ayew collected the rebound but he can't do anything with it.
3'
Fairly quiet start here as both sides move the ball around nicely. The Hammers are being given time on the ball but Swansea are doing well to keep shape when they don't have possession.
1'
The bubbles are flying around the London Stadium and we're underway. West Ham get us underway, kicking from left to right.
Good afternoon and welcome to the LIVE text commentary of the Premier League clash between West Ham United and Swansea City at the London Stadium, where both sides will be looking for a result to build upon after this week's international break.
West Ham are looking for their second home victory of the season after beating Huddersfield Town at the start of the month and Slaven Bilic in particular will be looking for a result, as his side have already lost four league matches this term.
Swansea find themselves in a similar position though, with their only victory this season coming two weeks ago against a Crystal Palace side rooted to the bottom of the table, a result they failed to build upon as they lost 2-1 to Watford last weekend.
West Ham XI: Hart; Zabaleta, Reid, Fonte, Cresswell; Antonio, Noble, Kouyate, Ayew; Hernandez, Carroll
West Ham subs: Adrian, Lanzini, Sakho, Ogbonna, Byram, Masuaku, Rice
Swansea subs: Nordfeldt, Fer, Narsingh, Clucas, Mesa, van der Hoorn, Rangel
Bilic makes two changes to the side that lost 3-2 to Tottenham Hotspur last week, with Angelo Ogbonna dropping to the bench and Marko Arnautovic out of the squad due to illness. The Hammers revert to a four-man defence, with Andre Ayew coming into mifield and Andy Carroll leading the line alongside Chicharito.
Swansea boss Paul Clement has also made changes, three in total, with his side also switching to a back four. Tammy Abraham, Renato Sanches and Leon Britton come into the line-up, which sees Mike van der Hoorn, Sam Clucas and Leroy Fer drop to the bench.
West Ham will fancy their chances against the Swans as they've lost just once in their last nine Premier League matches, a 4-1 result in May 2016. Andre Ayew was one of the men on the scoresheet that day and he now lines up for the hosts against his former employers.
Roger East brings the first-half to a close and neither side will be too happy with their performance. West Ham in particular have looked very poor on the ball but Swansea haven't offered too much going forward either. Goalless at the break and it's going to take a major improvement to change that.
If Slaven Bilic needed a performance to save his job, then this definitely isn't that. The Hammers have just 39% possession from the first-half and their only big chance came in the opening few minutes, when Michail Antonio's volley bounced towards goal and forced a diving save from Lukasz Fabianski. Excluding that chance, West Ham haven't really created anything.
Swansea have been the better of the two sides on the ball but they've also only had a few chances that have tested Joe Hart, notably Wilfried Bony's driven effort from distance. Renato Sanches has been good in certain spells but has lacked the consistency that held him back at Bayern Munich last season. Nevertheless, he looks to be the brightest player for the visitors.
Full-time at the London Stadium and it's a vital three points for the Hammers. Slaven Bilic will take the plaudits for a change that was controversial at the time, bringing Diafra Sakho on for Javier Hernandez. It proved to be decisive though as the Senegalese striker combined with fellow substitute Artur Masuaku to snatch the only goal of the game.
West Ham weren't at the races at all today and it's difficult to argue that they deserve the three points. There's plenty of room to improve for the Hammers but these points give them the perfect platform to build upon after the international break, where Bilic's side travel to face Burnley before hosting Brighton- two crucial games.
Swansea, meanwhile, will be caught in groundhog day after another late goal condemned them to defeat. They slip into the bottom three following this loss and have a lot of work to do when the Premier League returns in a few weeks.
